devices that can be used in the periphery as well. I am also competent in the use of the MacLab
monitoring system.

At this time I am also scrubbing many of the EP cases. The majority of which are PVI's and AV
node ablations.

Lead Radiologic Technologist, 2006 - 2012

Lead Radiologic Technologist in Cardiac Catheterization Laboratory. In charge of daily workflow for
a staff of 7 Registered Nurses and 3 Radiologic Technologists utilizing two rooms.

In the Cardiac realm I have experience with Diagnostic and Interventional Cardiac catheterizations
to include, LHC, RHC, BHC, PTA, Coronary Stenting (Rapid exchange and OTW systems), IVUS
(Boston Scientific and Volcano), Pressure wire (RADI and Volcano), Rotablater, Manual Aspiration
catheters, Angiojet, Datascope Balloon Pump, Impella, Pacemakers, Bi-Ventricular Pacemakers,
AICD's and Lead Revisions.

For the Peripheral realm I have experience with both Diagnostic and Interventional procedures.
This includes both Arterial and Venous systems. Procedures include, PTA, Stenting (Balloon
expandable and self-expanding), Silverhawk/Foxhollow devices, Diamondback Atherectomy,
Excimer Laser, Trellis, EKOS and various Vena Cava Filters. This includes many of the Cardiac
devices that can be used in the periphery as well. I am also competent in the use of the MacLab
monitoring system.

I have been in charge of ordering supplies at FMC for the past 6 years. I have worked closely with
the Materials management department to right size inventory in the lab. The last year I was in this
position I reduced the cost of our inventory by appx. $60,000.00.

I am competent in various routes of Venous and Arterial access. Femoral, Radial, Brachial,
Axillary, Popliteal, Dorsalis Pedis and in the past even assisted with Trans-Lumbar punctures.
Due to this, I would often obtain arterial and/or venous access during procedures for the
physicians.

I spent a lot of my 'spare' time in the lab educating staff and physicians on the various uses of the
equipment we had in our department. I would also spend a portion of my time sitting with the
physicians discussing and 'brain-storming' approaches and techniques for some of their more
difficult peripheral procedures.

Cath Lab Technologist, 2003 - 2006

Worked as a Cardiac Cath Lab Technologist. I had experience with a wide range of procedures
and equipment in the Cardiac realm. I performed Diagnostic and Interventional Cardiac
catheterizations to include, LHC, RHC, BHC, PTA, Coronary stenting, Volcano IVUS, Manual
aspiration catheters, Angiojet, Datascope Balloon Pump, Pacemakers, Generator changes, Lead
revisions and monitoring with the MacLab hemodynamics system.

I was initially involved in helping this lab move from a Diagnostic only to a full Interventional lab
taking acute MI's. I was given the duties of bringing in the supplies and inventory that would be
necessary to complete this task.

I was given the opportunity to learn about charge capture for our department. I helped set up a new
charging system for our department that increased charge capture and helped increase our
departmental revenue. I also learned how to access and submit ACC data to the national ACC
database.

Cath Lab Technologist, 2000 - 2003

Worked as Cardiac Cath Lab Technologist. I had experience with a wide range of procedures and
equipment. In the Cardiac realm I performed Diagnostic and Interventional Cardiac catheterizations
to include, LHC, RHC, BHC, PTA, Coronary stenting (Rapid exchange and OTW systems), IVUS,
Manual Aspiration catheters, Angiojet, Datascope Balloon Pump and MacLab hemodynamic system.

This was a lab that began moving from a Diagnostic only to an Interventional lab when I started. I
was able to help transition the lab staff in making this change by teaching the new staff the
process of coronary intervention and the acute MI setting.

I was also in charge of inventory management for this project. Along with the Director of the
department I brought in all the necessary equipment to stock the lab in preparation for coronary
interventions.

I received a PICC line certification during this time. I was able to perform PICC line insertions for
the Radiologists in the department utilizing the Sono-site vascular imaging system. As time
progressed I was able to train the PICC line nurses in the hospital to utilize this system. This
amounted to a substantial procedural savings for the hospital and eliminated the need to transfer
out the patients who had difficult access to our sister hospital.
