• Administering calls from Home Health & Hospice patients, caregivers, Insurance Companies, Hospitals , Outpatient Facilities, Doctor’s offices, DME companies , Lab Agencies and Wound Clinics

• Adjusting schedules to ensure fair level of productivity in visits for week. Adjusting productivity levels upon scheduling team request to accommodate patients and clinicians schedule for that particular day
• Tracking patients – Calling patients for new Starts of Care. Maintaining reports of patients who are hospitalized. Calling hospitals – verifying patient admission or observation - requesting transfer/oasis hospital admissions to be performed by clinician, communicating to Transition Liaisons at hospitals to receive Resumption of Care orders.
Calling patients to confirm home health patient has returned home after hospitalization
• Generating orders for New Starts of Care patients and Resumption of Care patients
• Generating reports for director for Daily Episodic Patients and for patients who have not been seen in 14 days
